Policies

Missed Appointments
     If the patient misses an appointment, patient will be charged a "No-Show" fee. The fees are: $5 for Tech Visits, $10 for any Office Visit. $15 for Driver's Physicals, Sports Physical or a Procedure, and $25 for a full Yearly Physical Exam.
     A reminder call may be made as a courtesy, but are not guaranteed.
     If you are unable to make your appointment we ask that you call 24 hours prior to your appointment time to cancel or reschedule.

     In addition, we ask that you call if you are going to be more than 15 minutes late for your appointment. The office will determine if we can still see you or if we must reschedule your appointment.


Insurance
     You must show your insurance card at every visit. Patients will be charged their "co-pay" on day of appointment. A service charge of $10 will be applied if the office needs to bill a patient for the "co-pay".
     Prior to scheduling any tests, please contact your Insurance Company to see if any referrals, notifications, or pre-certifications are needed.


Records Release
     If you would like your records released to another office, please allow ample time for records to be transferred.
     As always a signed release form is required to release records. A $30 (per patient) charge is assessed to copy and mail records to another Doctor's office. A $50 (per patient) charge for records being picked up by (or mailed to) a patient requires an office visit. It is required that payment be made prior to the transfer of these records.
     In addition, any past due balances must be paid prior to the release of your records.


Forms
     There may be a charge to fill out any forms that are not completed at your office visit.

 
Prescription Refills
     A minimum of 48 hours is required after the request of a medication refill. Refills for maintenance drugs will be provided if patient is up-to-date on their appointments. Narcotics will not be refilled without an office visit.
     Valley Medical Center does not participate in renewal of medications by FAX from any drug companies. The patient must resubmit their prescription to the company that they use.


Referrals
     Patients are responsible for filling out request forms for a referral. A minimum of 48 hours is required.

Payment
    
At time of visit, patient will be charged for services in full. Payment is accepted in cash, check, VISA, Mastercard, Discover, or American Express.
     Valley Medical Center does not currently offer payment plans.
