​Missed Appointments
Because of the burden that certain actions have on the practice, patients will be assessed fees in special circumstances.  Patients who fail to inform the office 24 hours in advance that they will miss an appointment shall be assessed a fee of $25.00*Multiple missed appointments may cause dismissal from our practice. 

If you arrive more than 15 minutes past your scheduled appointment, we will make every effort to
fit you, schedule permitting.
 
Insurance
Your insurance card must be present at every visit.  In addition, we will take your photo to validate your identity. This photo will not be shared with outside sources.
Prior to scheduling any tests, please contact your insurance company to see if any referrals, notifications, or pre-certifications are needed.  

Payment
Your co-pay is due on the day of your visit.

We accept payment in the form of cash, check, VISA, Mastercard, Discover or American Express.

Records Release 
A signed release form is required to release records. A $30 (per patient) charge is assessed to copy and mail records to another Doctor's office.  A $30 (per patient) charge for records being picked up by (or mailed to) a patient.  It is required that payment be made prior to the transfer for these records. Any past due balances must be paid prior to the release of your records. 

Forms
There may be a charge to fill out any forms that are not completed during your office visit.
